You will also want to document education, emotional support, and bereavement that was offered to the family. Notified coroner’s office of absent vital signs and body was ok’d to release to the funeral home.

Members of the IDG include: Medical Director Physicians Nurses (RNs and LPNs) Home Hospice aides Medical Social Workers Chaplains Bereavement Therapists (speech, occupation, physical) Volunteer Coordinator and Volunteers Patient and Family (are not required to attend meetings). You can also use the SBAR format when you call the physician.
