In 2020, the average length of stay for Medicare patients enrolled in hospice was 97.0 The median length declined slightly to 17 days, down from 18 in 2020. The question will be whether we start to see that longer length of stay as nursing homes and [skilled nursing facilities (SNFs)] get back after the public health emergency is over.

For a trial to have value, it should not exclude patients over age 80, or those with dementia, or patients residing in nursing homes. Don’t get me wrong, the evidence points to cost savings, but as Chris Callahan and Kathleen Unroe pointed out in a JAGS editorial in 2020 “in comprehensive dementia care models, savings may accrue to Medicare, but the expenses accrue to a fluid and unstable network of local service providers, patients, and their families.” Diane: Fantastic.
