1. Expert Phone Consultation

Give us a call and our dedicated client service specialist will be ready to listen to your needs, discuss options, costs and guide you to the next steps, all with no obligation and no cost to you.

2. In-Home Care Planning Meeting

When you’re ready to get started, our nurse will schedule an in-home meeting with you and any family members to assess the needs of the client. Collaboratively, you and the nurse will build a customized care plan, complete documentation, and consent forms, and discuss the schedule for care.

3. Team Collaboration 

Our goal is to deliver an individualized experience to each of our clients. Before services even begin, our staff members will work as a team to determine how best to meet the needs of your loved one. Our clients expect a successful and safe experience with Bayshore and our team is skilled at assessing what services need to be initiated to support our clients within and even beyond our scope of care.

Caregivers qualified to assist with the needs of your loved one will be selected, your care plan will be reviewed with them and services will begin.

4. Guidance & Monitoring 

Our manager will follow up with you on an ongoing basis to ensure your satisfaction. Open communication with our staff is strongly encouraged.
