CommuniCare Health Services Takes Over Operations of Transitional Care Company Stonerise

Ohio-headquartered CommuniCare Health Services recently took over operations of hospice and home health provider Stonerise, expanding the company’s reach in its home state as well as West Virginia. Financial terms of the move were undisclosed.

Stonerise’s transitional care model is designed to support patients and families across the health continuum. The company’s range of services and track record on quality made it an attractive asset, CommuniCare indicated in an announcement.

“The Stonerise brand is synonymous with serving through love, dedication and caring; and we are honored to welcome them into the CommuniCare family,” said Ronald Wilheim, CommuniCare president in the announcement. “They are known for their world-class facilities in their communities, and we are committed to continuing the level of quality clinical care and strength of operations for the residents and our employees at our new locations.”

Founded in 1984, CommuniCare Health Services is a family-owned company that provides post-acute services including skilled nursing, assisted living, independent living, behavioral health and long-term care.

Stonerise provides hospice, home health, transitional and skilled nursing care, and therapy services to more than 2,000 patients across West Virginia and Southeast Ohio. The company was established in 2009.

The company will continue to operate under the Stonerise brand. Its 17 health care centers and seven home health, hospice and affiliated therapy agencies widen CommuniCare’s existing footprint in both states.

Hospice utilization among Medicare decedents in West Virginia reached 43.6% in 2018, according to the National Hospice and Palliative Care Organization. This fell below the national average of 50.3% that year. Meanwhile, Ohio’s hospice utilization rate climbed above it at 56.7%.

CommuniCare now operates more than 110 health care centers across seven states. 

The acquired company’s 3,000 employees will transition to CommuniCare, according to Stonerise CFO Jessica Hudson. Post-closing, Hudson will be the division vice president overseeing CommuniCare’s health centers in West Virginia.

“CommuniCare and Stonerise hold many shared values and working together, we will continue to provide quality care to those patients we serve,” said Hudson in the announcement. “I also am excited about CommuniCare’s commitment to retaining all Stonerise team members and creating opportunities for new team members in West Virginia.”
