Our care teams are made up of a wide range of professionals and volunteers who care for the whole person, which means that we recognize and address emotional, mental, physical, and spiritual needs. Members of our care teams include:

  • Physicians
  • Skilled nurses
  • Medical social workers

  • A bereavement support team
  • Spiritual professionals
  • Volunteers
  • Therapy services as indicated

We’re passionate about building relationships with patients and their families. We know that the time we spend with our patients and their loved ones is sacred, and we’re here to provide any necessary support at every stage.

Joint Commission Accreditation

Joint Commission accreditation is the gold standard for home health and hospice providers, and we’re proud to have earned this distinction. Accreditation surveys are conducted at least once every 36 months by a team of highly trained healthcare professionals, to ensure compliance with the highest standards. These unannounced surveys involve a thorough evaluation of patient care and safety through direct observations, staff interviews, and patient discussions, verifying that our commitment to excellence is more than just a goal—it’s an integral part of our daily operations.

Joint Commission accreditation isn’t mandatory. It’s a voluntary commitment that we’ve made to consistently provide the highest quality of care. Our accreditation demonstrates our dedication to meeting and exceeding industry standards for patient care, safety, and outcomes. Patients can receive personalized care from passionate professionals at 43 Joint-Commission-accredited hospice branches, and we’re continually working to achieve accreditation for all Traditions Health hospice locations.

We Honor Veterans

We are proud to partner with We Honor Veterans in more than 70 branches across the country. This program honors and supports our nation’s veterans in their end-of-life journey. Veterans have unique needs when nearing the end of life, and our team is trained to address these needs with compassion and respect.

Through the We Honor Veterans program, a collaboration between the National Hospice and Palliative Care Organization and the Department of Veterans Affairs, Traditions Health is better equipped to address the unique end-of-life needs of veterans with respect, compassion, and acknowledgment. Whether by giving veterans an opportunity to tell their stories, assisting with Veterans Affairs benefits, holding pinning ceremonies, or connecting patients with veteran-to-veteran volunteers, we are committed to ensuring that our nation’s heroes receive the care and support they deserve in their final days.

ASAP (Anticipatory Symptom Assessment Pathways) Program

Proactive and preventative care for our patients and their caregivers is a top priority. That’s why we have developed the Anticipatory Symptom Assessment Pathways (ASAP) program, a service designed to support both patients and caregivers by equipping them with the tools and knowledge they need to manage symptoms and reduce anxiety during critical times.

An easy-to-use symptom management guide helps patients and caregivers identify and manage symptoms right away. Additionally, 24/7 access to an expert hospice care team provides additional symptom management support as needed.

Specialty Programs

At Traditions Health, we understand that each patient’s journey is different. That’s why we offer specialty programs for those with specific diagnoses. Our expert team members have specialized training in caring for patients with Alzheimer’s, dementia, cardiac disease, respiratory disease, ALS, cancer, and more. Our patient-focused approach ensures that every patient receives the specialized care they need.

Benefits of In-Home Hospice Care

Why choose in-home hospice care over other end-of-life care options? The answer is simple: quality of life. Hospice care brings comfort, dignity, and support to patients in the last months of their lives. The end of life should be about living the way you choose. That’s why at Traditions Health, we care for the whole person, providing support and comfort to patients in the place they call home.

Just a few of the benefits of in-home hospice care include:

  • Symptom management to help patients remain in their home environment as long as possible
  • Reduced hospitalizations, physician visits, and emergency room trips, relieving stress for both the patient and their loved ones
  • Emotional and spiritual support for patients and their families, helping them through the end-of-life journey
  • Equipment and medication management to ensure safe and effective care in the home
  • Personalized care plans that consider each patient’s circumstances and preferences
  • Support for caregivers to prevent burnout and allow them to focus on spending quality time with their loved ones

While hospice care can be provided in various settings, including hospitals and long-term care facilities, the comfort and familiarity of home cannot be equaled. For many patients, being surrounded by familiar belongings and loved ones provides a sense of peace and stability during this difficult time.

Choosing a Hospice Care Provider

Selecting a hospice care provider requires a careful evaluation combined with a reliance on the heart. It’s about more than vetting accreditations and services. It’s about uncovering values that echo your personal ones and the values of your family.

Length of service and positive testimonials from families are essential factors to consider when choosing a hospice care provider. Does the provider have a long-standing history of providing quality end-of-life care? Are the loved ones of patients satisfied with the care provided? These questions can help you gauge the level of trust and satisfaction a hospice provider has earned.

Traditions Health’s unwavering commitment to excellence and compassion make us a trusted choice for in-home hospice care. While we’re accredited by the Joint Commission in 43 locations and have received many recognitions for excellence in the field, the testimonials of our patients and their loved ones speak to the quality of care we provide.
