CMMI extended the program through 2030, but recently announced that the hospice portion will end on December 31, 2024.

A new collaboration for transitional care between Florida-based Big Bend Hospice and the Tallahassee Memorial HealthCare system has more going on behind the scenes. Big Bend, a nonprofit provider, in 2021 formed a new parent company called Seven Oaks Healthcare. Certainly the idea is to take pretty big leaps towards value-based care.

The hospice, palliative care and personal care provider Gentiva has acquired the hospice, home health and palliative assets of the nonprofit health system ProMedica. Though Gentiva did not disclose the financial terms of the transaction, Bloomberg reported that ProMedica’s Heartland Hospice is valued at $710 million.

The hospice and senior care provider Helios Care is partnering with the integrated health system Bassett Healthcare Network to bring more extensive access to hospice and palliative care to patients in central New York. Formerly known as Catskills Area Hospice and Palliative Care, the organization rebranded as Helios in 2019.

Maine-based Androscoggin Home Healthcare and Hospice has rebranded as Andwell Health Partners to better reflect its growing suite of services.
